For clinics and health practices
Patients sign intake and consent forms while they wait.
Patients fill in the intake form and sign the consent on the clinic’s tablet. Each signed form is kept as a sealed PDF with its history. Maple eSign is Canadian-owned and hosted in Canada. It’s free to use, and you can sign up online, for yourself or your team.
In-person signing
The front desk hands the tablet to one patient at a time.
Add the patient as an in-person signer, with someone at the front desk as the host. The host must be signed in to Maple eSign as an owner or admin. The patient signs on the clinic’s tablet, and the host is named in the record. When the session is closed or runs out of time, the tablet stops taking input and the screen is cleared.

Templates
Set up each clinic form once and reuse it for every patient.
Save your intake form, consent to treatment and release of records as templates. The fields stay where you put them, and conditional fields show a question only when it applies.

Before we switch
- Read our college’s standards on consent and on medical records.
- Check our province’s health privacy law. Ontario’s, New Brunswick’s, Newfoundland and Labrador’s and Nova Scotia’s are declared substantially similar to PIPEDA. The other provinces have their own.
- The app, its database, document storage and backups run in Google Cloud’s Montreal region.
- Email goes through Mailgun, in the United States. It carries names, email addresses, the document title and the link, so we keep health details out of titles.
- Google sign-in is optional, and the public website runs on Cloudflare’s global network.
